Dental Coverage for LA HAP members
Who is eligible for the plan?
The plan is open to all LA HAP members who do not have another dental plan through their employer or Medicare. Members who do have an employer or Medicare plan can choose whether they want to keep their current plan or switch to the LA HAP/Guardian plan.

How does the plan work?
First, you find a provider in the Guardian network. To do this, call Guardian at 1-800-541-7846 and ask about their PPO network. Or, you can search online:
- Visit www.guardiananytime.com.
- Click “Find a Provider” from the top menu.
- Click “Search Providers.”Make sure you are on the “Find a Dentist” tab. Select “PPO as plan type,” and search your zip code within a radius of 2-200 miles.
Once you make an appointment and visit the dentist, you will show them your Guardian insurance card or give them your member information. They will send the bill to HIP for your visit.
If you go to a dentist who is NOT in the Guardian Provider network, or if you get a service that is NOT a Guardian-covered service, you may have to pay some costs yourself. It’s important to know what is covered under your plan.
What about my current dental plan?
If you already have a group dental plan through your employer or a supplemental dental benefit through Medicare, you can either keep that plan OR enroll in the LA HAP/Guardian plan.
LA HAP/HIP does not cover the costs of other standalone, individual dental plans (“Individual” means a plan that is NOT tied to your job or to your Medicare plan).
